Services and Fees

We accept CASH only. All expenses in connection with the examination must be paid by you at the time of the examination.

Insurance is not accepted for these physical.



Our fee is $170.00 for Children (0 to 14 Yrs of Age)
And for 15 Yrs of age and older is $220.00


The Services included are:

  • Basic Pshysical Examination
  • Required Blood work, Syphilis
  • Tuberculosis skin tesing
  • Vaccination status verification
  • Copies of your laboratory results
  • Certified and Sealed I-693 Form
    Can be Downloaded by Clicking here
  • Chest X-ray, if needed
  • Minor paper work you have to fill out.
  • Please note: If any vaccinations are needed
    those will be at extra cost
 

 

The whole physical exam process normally requires one week if everything goes well.

Citizenship Immigation Physicals Requirements:


Testing Needed for a Adult examination:

  • A Physical Examination
  • A tuberculin (TB) skin test.
  • Blood test for HIV
  • Blood test for syphilis (RPR)
  • Immunizations are dependant on age and medical records.

The following tests are required by the US Citizenship and Immigration Service, or USCIS (formerly the Immigration & Naturalization Service, or INS).

 

The physical examination is required for persons of all ages. The examination requirements are determined by your age and your immunization record.


Needed Immunizations and Information:


Certain immunizations are required by the USCIS. The requirements have changed as of 1 July 2008. If you are not able to produce acceptable proof that you have had the immunications or the diseases themselves, you may have the immunizations administered by our Business Health Travel Program, your health department or personal physician.


A Pedatric Constolation of a Child 14 and younger have the follwoing Examinations.

  • A physical examination
  • A TB skin test(2 years of age and older)
  • A review and update of the pediatric immunizations.

Adult Immunizations Are as followed:

  • Measles, Mumps, Rubella vaccine
  • Tetanus/Diptheria vaccine
  • Varicella (Chicken Pox) vaccine
  • Influenza (during flu season)
  • Zoster vaccine (for those over age 65)
  • Human Papillomavirus vaccine for women age 11 to 26

Childhood Immunizations depend on age and may include:

  • Rotavirus vaccine
  • Measles, Mumps, Rubella vaccine
  • Tetanus, Diptheria, Pertussis vaccine
  • Polio vaccine
  • Haemophilus Influenzae Type b vaccine
  • Hepatitis B vaccine
  • Hepatitis A vaccine
  • Pneumococcal vaccine

What you need to bring with you:

  • Picture identification such as a passport or state identificaion, a driver's license.
  • Your medical records for any chronic or serious medical problem.
  • Your immunization records, childhood records, if available.
  • Alien Registration number if you have one (starts with an "A").
  • Haemophilus Influenzae Type b vaccine
  • Hepatitis B vaccine
  • Hepatitis A vaccine
  • Pneumococcal vaccine
